Should you be unlucky enough to ingest contaminated food or water, the vomiting and or diarrhea are more of a threat to your health than the particular pathogen, whether it be a virus, bacterium, or parasite. Mild diarrhea in adults can be managed with Imodium available overthe-counter ; or Lomotil available by prescription ; . However, diarrhea drains salts and fluids from the body, causing the dehydration that accounts for the deaths of babies worldwide. While an adult's diarrhea is not usually life threatening, grown-ups should replace the salts and fluids they are losing with almost any fluid. If diarrhea is severe more than one stool every two hours ; , adults should use an oral rehydration solution ORS ; such as Pedialyte. An ORS powder to be mixed with boiled water is available over the counter at pharmacies around the world. Should you or your child feel ill the first year after your trip, alert your doctor as to which country you visited. The doctor will then order the appropriate series of stool examination kits. Most diarrhea-producing parasites such as Crypotosporidium or Giardia are transmitted by contaminated water. Even if the diarrhea abates, you may still be harboring the parasite. Get rechecked at home if you are not completely back to normal. Other parasites, such as tapeworms and flukes, are transmitted by contaminated foods and, occasionally, by water. Some, like hookworms and schistosomes, which cause schistosomiasis, are acquired through the skin by walking barefoot outside or swimming in contaminated water. Most of these parasites require an intermediate animal host and are not contagious from person to person. These parasites typically do not give diarrhea and may be detected only months or years after travel. Since most parasites are transmitted in a food-fecal chain, consider each diaper a transmitter. Make certain that all members of your family keep up a scrupulous hand-washing ritual. All orphans and their new parents with symptoms should be tested for parasites after their arrival home. Some worms and parasites have dormant periods and will not show up in every stool sample. You or your child should be rechecked if symptoms persist. Chapter 13 provides a more detailed overview of the potential health problems of children from developing countries. ; Most bacterial pathogens, such as Salmonella and Shigella resolve on their own. There is no need to recheck after arrival unless symptoms persist or new problems develop. A few viral pathogens may be spread by travelers after arrival home, especially Hepatitis A and Hepatitis B. Of course, strict hand washing should be practiced even after arrival back home. An ill person should not prepare food for the family, nor share food or utensils with anyone.

Limitations. Optical mapping is well suited for measuring Vm on the epicardial surface of intact hearts. However, Vm could not be measured from deep layers of myocardium, and hence the contribution of these layers to the complex three-dimensional nature of cellular coupling could not be investigated using this technique. More importantly, fiber direction is known to change across the ventricular wall, thereby affecting cell-to-cell coupling in that direction. Moreover, in this study, no major structural discontinuities, such as those introduced by fibrotic lesions near the border zone of an infarction were present. The presence of such discontinuities may influence the measurement of ST-Vm and hence ; by violating continuous cable theory. 4 In 1986, Kligman et al6 reported cosmetic improvement of photoaged skin of the face and forearm with topical tretinoin. Initially, their assertions that tretinoin could reverse aging were met with some skepticism. This could be attributed to the difficulty in distinguishing differences between photoaging and chronologic aging by noncritical observers. The clinical changes reported by Kligman et al were supported by histologic and ultrastructural reversal of human photoaging. There were specific changes documented in the epidermis as well as the dermis. A number of subsequent double blind, vehicle controlled studies of human photoaged skin7, 8, 9, 10 have confirmed tretinoin's benefits and provided ultrastructural details and other supporting data for reversal of photoaging. Consequently, tretinoin has been approved for this specific use in the United States and in several other countries.

There are traditional healers and herbalists all over the world, and many have particular specialties. Refer to the list of centers in Resources at the end of this article sorry not included here ; , either before you arrive or if you become ill. They will be able to put you in contact with local, reputable, natural healers. Eat garlic. Garlic is a natural remedy for treating infections due to bacteria or parasites. Albert Schweitzer is said to have used garlic in Africa to treat Amoebic dysentery and laboratory tests have shown garlic juice diluted to one part in 125, 000 inhibits the growth of staphylococcus, streptoccoccus, vibrio cholera, and the bacillus associated with dysentery. Carry packets of freeze-dried miso, or a container of regular miso so you can replenish intestinal flora if you become ill. Umeboshi in its various forms is also helpful. Dehydration. One of the greatest dangers off diarrheal disease is dehydration. Dehydration poses the greatest threat to infants and young children, but anyone can become dehydrated, especially when malnourished or chronically ill. As soon as diarrhea begins you must start oral rehydration therapy. Oral Rehydration Salts ORS ; are available in most health centers and pharmacies in the less developed world, but if you are unable to obtain the prepared packets you may make your own. The recipe is: 1 Quart boiled water with cup orange or lemon juice if available ; 2 Tablespoons honey teaspoon sea salt teaspoon baking soda optional ; Drink two glasses after each elimination, and up to 3 quarts a day. Infants should continue to be breastfed and fed up to a quart of the solution daily. Children receive 2 quarts daily. When it is possible to eat food, choose foods that are rich in potassium such as mashed bananas, papaya, coconut, and orange juice. Brown rice and cinnamon provide bulk and protein and the cinnamon acts as an astringent, helping to cleanse the intestinal tract and reduce gas. Continue to drink the rehydration fluids as long as diarrhea persists. Many drugs are available over-the-counter in Third World countries; many are useless and even dangerous. The oft-prescribed drug Lomotil diphenoxylate and atropine ; is one such drug. Lomotil slows intestinal spasms but does nothing about the cause of the diarrhea. If the spasms are painful, take 20 drops of tincture of valerian. In children, Lomotil may stop normal intestinal contractions and result in serious or fatal respiratory depression. Another commonly available medication, Entero-Vioform iodochlorhydroxyquin ; has been banned by many countries yet remains on the shelves of others. Entero-Vioform is an anti-diarrheal drug which has been linked to sub-acute-myeloneuropathy, which can cause paralysis and blindness. Under no circumstances should you take this drug. Another drug, which is commonly used in the Third World, is called Flagyl, or metronidazole. It is prescribed in the U.S. for trichomonas and throughout the world for amoebiasis. Side effects may include nausea, headaches, and diarrhea. Flagyl should never be used by pregnant women or people with a history of blood disease. Animal studies have pointed to the mutagenic effect of Flagyl in doses equivalent to those given to women for trichomas or ameobic dysentery. If you know that you have parasites or once you return have symptoms, obtain ParaGuard or Black Walnut extract. By following many of these simple, hygienic procedures, you will have increased protection against many of the infectious diseases common in the Third World. Keep in mind your reasons for travel and of prime importance is the challenge to harmonize with the people, culture, and environment. Culture shock is a real phenomenon, which may manifest as irritability, disorientation, and depression. Travel slowly, sleep a lot, eat lightly and drink plenty of juices and purified water. Illness is often a transitional state that requires that we slow down and take stock of our movements. Though you may be traveling through economically poor areas, they are often culturally rich and shake us out of our insularity and chauvinism. Thus, when the local population takes a break at mid-day, for food and rest, join in the custom, for as the old adage goes: when in Rome. and it remains good healthy advice and lomustine.
