Tions of valvulopathy improved with cessation of pergolide therapy. Two patients required valve replacement. Indications: Pergolide mesylate, a dopamine receptor agonist, is used with levodopa or with a carbidopalevodopa combination to treat patients with Parkinson's disease, a progressive neurological disorder resulting from the degeneration of neurons in a region of the brain that controls movement. Pergolide mesylate works by stimulating certain parts of the central ner vous system that are involved in this disease. In deciding whether or not to use pergolide mesylate, prescribers must weigh the risks associated with the drug against the benefits it confers. On further analysis, the risk appeared to be greater in African-American patients. In white patients 71% of the study population ; , there were no significant differences between treatment groups for primar y events and asthma-related events. In African-Americans 17% of the trial's enrollees ; , the study showed a statistically significant greater number of primary events and asthma-related events, including deaths, in patients taking salmeterol than in those taking placebo; however, fewer than 1% of all African-Americans enrolled in the study experienced such events during the 28-week trial. In addition, patients who were not taking inhaled corticosteroids at the time of enrollment appeared to be at greater risk for serious adverse outcomes than those who were taking inhaled corticosteroids. Indication: In 1994, salmeterol xinafoate inhalation aerosol was approved to treat asthma. This approval was later extended to treat chronic obstructive pulmonar y disease COPD ; , including emphysema and chronic bronchitis. The drug belongs to the class of asthma medications known as 2-receptor agonists commonly called beta agonists.
Fahey T and Howie J. Re-evaluation of a randomized controlled trial of antibiotics for minor respiratory illness in general practice. Family Practice 2001; 18: 246248. Background. A systematic review examining the efficacy of antibiotics in acute respiratory illness concluded that antibiotics are of little benefit. However, that review was based on analysis of only six randomized controlled trials, one of which was excluded because its analysis included patients with multiple episodes of illness; treatment group, either antibiotic or placebo, might have confounded the likelihood of suffering a subsequent episode of illness. Methods. This previously excluded randomized controlled trial of 301 patients with symptoms of minor respiratory illness was re-analysed to examine the efficacy of antibiotic versus placebo in terms of resolution of symptoms, most particularly cough. Results. Antibiotic had no impact on the resolution of symptoms of cough at 1 and 2 weeks, respectively; adjusted odds ratio 1.2 [95% confidence interval CI ; 0.72.1] and 0.8 95% CI 0.41.6 ; . In those 220 73% ; individuals who suffered a cough, 48 44% ; and 19 17% ; of patients taking placebo were still coughing after 1 and 2 weeks, respectively. Conclusion. It appears that an antibiotic is likely to have, at best, a marginal impact on resolution of symptoms for most patients with minor respiratory illness in the community. Keywords. Antibiotics, cough, respiratory illness.

Since most common antidepressants act on catecholamine pathways, we performed a subsidiary analysis that excluded these patients patient #1, 8, 10, 12-14; Table 1 ; . HR, MAP, and FVC responses to LBNP trials and mental stress in the patients not taking antidepressants were similar to all patients and none of the psychological scores was correlated with the changes in HR during LBNP or mental stress in this subset. Additionally, Anxiety Sensitivity Index scores in these patients were significantly higher than the controls P 0.05 ; whereas Body Vigilance Scale and Catastrophizing Scale scores were not P 0.1.

People with Parkinson's taking pergolide Permax ; may be aware that the drug has come under scrutiny recently for a previously unrecognised side effect. However the study in the United States only involved around 200 patients so those in New Zealand taking pergolide should not stop taking the drug but should consult their doctor for advice. Researchers from Texas presented evidence at the recent American Academy of Neurology 56th Annual Meeting in San Francisco, California that pergolide use may be associated with an increased risk of heart valve disease. Wellington neurologist Edward Wong said, "Although recent reports have highlighted this issue, the fr e q importance of this complication is still to be determined. Most neurologists and physicians in New Zealand will be aware of this problem, and people concerned about this should discuss it with their doctor." Journalists and newspapers have performed a great service by focussing on the short-lived "Muta marriages" of Bombay and Hyderabad, an aspect of "Prostitution Tourism". Journalist Kalyan Chander Jaiswal explained how these so-called "marriages" are contracted and how young girls are exploited, for the men quickly walk out on their "brides". In so doing, the journalist blasted the myth that the husbands from the Gulf countries are all wealthy sheikhs. Indeed, most of the men claimed they were important officials or rich men when, in fact, they were merely security guards, chauffeurs, petty housekeepers and the like. Even for a rich man, though, a "muta marriage" for a few weeks is a better bargain than having to pay for a call girl every night. What is more, the girls are often virgins, and this means they can command a higher price. The journalist cited cases of girls who were "married" to men two or even three times their age for a sum of 3, 000 rupees and who were then deserted. Take, for example, the case of 13-year-old Zubeida, the daughter of a deceased car mechanic. Her widowed mother was tempted by the money and sold her daughter to a 73year-old man from the Arab Emirates. He spent eight days with the girl, had a passport made out for her, and then disappeared. Now she and her sisters, who had never worn the chador before, do so to hide from the taunts of their neighbours. This is typical of the poorer families, who are sorely tempted by the prospect of a dowry.

Our total international long distance service revenues decreased by Php559 million, or 4%, to Php12, 245 million in 2005 from Php12, 804 million in 2004 primarily due to the peso appreciation coupled with the decrease in call volumes largely as a result of alternative means of communications such as e-mailing, texting and Internet telephony. The percentage contribution of international long distance service revenues to our fixed line service revenues decreased to 25% in 2005 from 26% in 2004. Our revenues from inbound international long distance service decreased by Php249 million due to a decrease in inbound traffic volume by 113 million minutes to 2, 079 million minutes in 2005. In addition, the 2% appreciation in the average value of peso to U.S. dollar to Php55.085 during 2005 from Php56.044 during 2004 contributed to the decrease in our inbound international long distance revenues in peso terms since settlement charges for inbound calls are billed in U.S. dollars or in special drawing rights, an established method of settlement among international telecommunications carriers using values based on a basket of foreign currencies that are translated into pesos at the time of billing. Our revenues from outbound international long distance calls in 2005 decreased by Php310 million, or 13%, to Php2, 042 million from Php2, 352 million in 2004. The decrease resulted from lower conversion rates used as average billing rates were Php55.30 and Php55.98 in 2005 and 2004, respectively. Our outbound international long distance call volumes declined by 6% to 147 million minutes in 2005 from 156 million minutes in 2004, primarily due to cellular substitution subscribers opting to use cellular for international outbound calls ; and the popularity of alternative means of communications such as e-mailing, international text and phenylephrine.

Of the differences was statistically significant.32 Further studies will need to clarify whether the difference between these 2 studies implies that among HIV-negative women the additional benefits of monthly dosing only become apparent when sulfadoxinepyrimethamine resistance is well established eg, 25% by day 14 ; . Two such studies comparing the 2-dose regimen with monthly IPT with sulfadoxine-pyrimethamine during pregnancy are ongoing in Malawi and Tanzania and the results will become available later in 2007 and in 2008. This is the 49th issue of Perspectives magazine which has evolved over 14 years of publication to its present electronic format. As an e-zine, e-Perspectives will be available to a wider and wider audience of healthcare professionals who value the substantive original articles we publish. e-Perspectives will now be available worldwide to anyone with an Internet connection. The PDF articles may be downloaded and printed by those who want a print copy for reference. In this issue of e-Perspectives, Sidney Moormeister leads with an inspiring article on the need for integrity in the medical transcription profession: "Would You Sell Your Soul? and How NOT to Make a Workplace Faustian Bargain ; ." Randy Drake launches a new column on pharmaceuticals, "Dear Drake and Drake: Q&A Rx and OTC." Randy and Ellen Drake, authors of the annual Saunders Pharmaceutical Word Book, share their expertise on prescription and over-the-counter drugs. With his characteristic dry wit, Randy tells us right off the bat "Google doesn't spell anything, " reminding us it is search engine and may pull up thousands of misspellings. Renee Priest, in "The Hip Bone's Connected to the Thigh bone, " delights us with her clever depiction of how dictating stations for physicians are established. Richard Lederer effectively makes the point that English is the universal language. Joe Weber comes the closest he's ever been to admitting that speech recognition is NOT going to replace medical transcriptionists in our lifetime, which he has long predicted; he describes the process of "back-end recognition" as one that makes use of medical transcriptionists as medical editors and, in some cases, may increase productivity and reduce costs. Featured in this first issue of e-Perspectives is an article by John H. Dirckx, M.D., "Urines Are Cooking: Perspectives on Medical Slang and Jargon, " with a two-page spread of terms, "Translation, Please! Medical Slang and Jargon." The article grew out of our recent translation efforts while producing the new course, The SUM Program Beginning Medical Transcription, 2nd edition. Dictating physicians use so much medical slang and jargon in their dictation that medical transcriptionists are constantly called upon to know which terms to transcribe verbatim, which ones to translate into formal terms, and which ones to flag. Dr. Dirckx offers a classification of medical slang and suggests guidelines for its management. Rounding out e-Perspectives is the three-page "Update: What's New in Medicine" by Ellen Drake. These and other new terms will appear in Vera Pyle's Current Medical Terminology, 10th edition, to be published in Spring 2005.
